Three rock climbers were killed over the weekend in a climbing accident in the North Cascade mountains in Washington state, as investigators look into equipment failure.

The Okanogan County Sheriff’s Office, along with local search and rescue teams, received a call about a climbing accident that took place inside of North Cascades National Park. The area where the climbers fell is known as North Early Winters Spire,
